Major differences between the Church of Scientology's sauna and that of medical doctors are that 1 ; The Church places a partner in the sauna with you who has already been through the experience, and assures that you are experiencing everything OK; and, also the Church has a supervisor review your log of daily events; 2 ; Unlike the Church, Medical doctors usually take laboratory samples that report on specific xenobiotics and these will be compared against progress in the decrease of your chronic symptoms. This sauna treatment requires strong will for continued exposure and endurance, but, once you've gone through the initial "want-to-quit" stage, you'll find it easy to endure, and quite beneficial, even restful. 23. After doing all of the above will I be well? No one knows the answer to such a question! Keep in mind that you're the camel, and your back is being weighted downward. The key principle to wellness is to began removing the straws that hold you down. How many straws there are, and whether or not you actually remove them is between you and your health professionals. No one -- other than you -- knows if you've given each straw an honest tug. Then, too, there may be other straws that we've not mentioned, or we've not known about. One such, for example, might be problems specific to you such as Diabetes type II normally can be traced to serious food allergy problems cancer a serious systemic and metabolic disease; the tumor is not the cancer! ; , long-standing metallic poisoning from sources we've not mentioned, and so on. Of course if you're one of those who've been given a patented drug to alleviate a symptom, and then another to alleviate the side-effects of the first drug, and then another to suppress the side-effects of the second drug -- ad infinitum -you've been long-conned into the patented drug game which fattens the portfolio of pharmaceutical companies, bottom-line "health" insurance agents and unthinking doctors! In your drugged state of apathy and slow thought you probably don't have much opportunity to become the lead pack!

Costeroids have benefits as well as adverse effects. Candida albicans infections of the mouth and pharynx can occur and lead to white plaques inside the mouth, cottony feeling in the mouth, loss of taste, and pain when eating or swallowing.19 These infections can spontaneously resolve or require treatment. This is why it is important to counsel patients to rinse and spit after each dose of inhaled corticosteroids. When using nebulizer systems, patients need to rinse their mouths and clean the skin around the mouth to avoid fungal infections. Potential adverse effects of inhaled corticosteroids include increased risk of osteoporosis. In a 2003 meta-analysis, the authors concluded that patients taking high doses of inhaled corticosteroids who have other risk factors for osteoporosis should have their bone mass density closely monitored.20 Preferred treatment for moderatepersistent disease is to increase to medium-dose inhaled corticosteroids and add long-acting inhaled beta2 agonists. Longacting inhaled beta2 agonists include salmeterol Serevent ; and formoterol Foradil ; . These agents relax bronchial smooth muscle by selective action on beta2 receptors with little effect on heart rate. Improvements in FEV1 are seen.

Increased from 5% in Group A patients to 42% in Group B patients. Notably LMA use increased 21 fold. The author's aggressive approach of incorporating the ASA Difficult Airway Management guidelines by early intervention with accessory airway devices lead to a remarkable reduction in multiple attempts at laryngoscopy and a decreased incidence of airway and hemodynamic complications. This study confirms the importance of application of the ASA Difficult Airway Management algorithm outside of the operating room setting and also justifies the immediate availability of a well stocked Difficult Airway Cart in all hospital locations where emergency airway management is performed, especially the ICU. The second study reviewed the utility of exchanging an endotracheal tube in the ICU by two methods: direct laryngoscopy or airway exchange catheters 26 ; . Endotracheal tube exchanges from an eight year QI Database were reviewed. Patients with an uncompromised glottic view Cormack-Lehane view 1&2 ; were divided by method of exchange Direct Laryngoscopy vs Airway Exchange Catheter-Cook 14F or 19F ; . Hypoxemia, intubation attempts, esophageal intubation, bradycardia, cardiac arrest and the need for a surgical airway were compared. The author studied 133 patients with an uncompromised glottic view DL 99, AEC 34 ; . Successful endotracheal tube exchange on the first attempt was higher with use of an AEC 95% AEC vs 62% DL ; . Need for multiple attempts at laryngoscopy was higher with DL 26% DL vs 2.9% AEC ; . Rescue airway techniques were utilized more frequently in the DL group 16 cases, 5 surgical airways DL group vs none of the AEC group ; . Hypoxemia and severe hypoxemia was more common in the DL group as was esophageal intubation. Bradyarrhythmias and cardiac arrest during DL for endotrachal tube exchange were also more frequent. The author concluded that use of an AEC during endotracheal tube exchange in the ICU lowered the risk of complications considerably even in the presence of an uncompromised view of the glottic inlet. Critically ill ICU patients often require diagnostic or surgical procedures that traditionally require transportation outside of the ICU. The risks inherent in the transport of critically ill patients are well described. A recent article by Waydhas reviewed the current literature on the intrahospital transport of critically ill patients 27 ; . Adverse events can occur in up to 70% of critically ill patients during transport outside of the ICU. These include hemodynamic compromise Bradycardia, Tachycardia, Hypotension, Hypertension, Arrhythmias, Cardiac Arrest ; as well as respiratory compromise Tachypnea, Hypocapnia, Hypercapnia, Hypoxemia ; . In 12% of cases reviewed, long term deterioration of respiratory status was observed. In one-third of cases, mishaps during transport were equipment related, for example, sreevent diskus inhaler.

Introduction: Incidence of renal artery stenosis detection in diabetic hypertensive patients while under going coronary angiogram is well studied. But additional obstructive coronary artery effect on renal artery stenosis detection has limited data. We want to study the incidence of renal artery stenosis requiring revascularization in stable coronary artery diseased patient's. Methods: chronic stable angina patients undergoing coronary angiograms were included in this study prospectively. coronary angiogram done for actue coronary syndromes or done for separetely only to detect renal artery stenosis were excluded. Results: Total number of cases undergone renal angiogram along with coronary angiogram indication was coronary artery disease ; over one year 2005nov to 2006 nov ; in our institution were 221. Out of them 151 patients 68.3% ; had both diabetes and hypertension and 70 patients were without diabetes 31.7% ; but had other coronary risk factors hypertension, smoking and dyslipedemia ; . In 53 patients 23.9% ; renal artery disease was detected by routine added renal angiogram along with coronary angiogram. Out of these 53 patients , 26 patients 11.8% ; had renal artery stenosis was 70% and radionucleide renogram demonstrating significance of this stenosis. These 26 patients were treated with renal angioplasty. 12 patients 5.4% ; had insignificant obstructive coronary artery disease but renal artery abnormalities were detected. Out of them 70% renal artery stenosis was present in 7 patients 3.2% ; and 70% in 5 patients 2.3% ; . Out of 12 patients with renal artery abnormalities 5 patients doesn't have diabetes mellitus 2.3% ; . The incidence of renal artery abnormality without diabetes but requires revascularization renal artery angioplasty ; was 0.9% two patients ; . The incidence of renal artery stenosis is 23.98% in patients with stable coronary artery disease, along with classical coronary risk factors. In insignificant obstructive coronary artery diseased nondiabetic patients the incidence of renal artery stenosis requiring revascularization comes down to 0.9%. Conclusion: In conclusion, it is better to add additional renal artery angiograms to conventional coronary angiogram in obstructive coronary artery diseased diabetic hypertensive patients, but not in insignificant obstructive coronary artery diseased nondiabetic patients.
