THE EXPANDING ROLE OF NURSES The huge changes in our socio-economic environment and population profile in recent decades have had major implications for healthcare service providers. We have moved from a healthcare system where roles were more clearly defined to one where roles have expanded and developed to meet these changes. Accordingly, nurses who work in dynamic healthcare environments are experiencing role expansion at an accelerated rate. It is vital that every nurse is clear as to her scope of practice and understands the implications of her nursing activities both in terms of patient care, and in terms of the legal implications of her actions. This is particularly true for practice nurses working in general practice where, all too often, the lines of professional responsibility can become blurred. PROFESSIONAL BOUNDARIES To understand the legal implications of extended roles it is important to be familiar with the appropriate standard of care in any particular situation. In the English decision of Wilsher v Essex Area Health Authority 1986 ; , the plaintiff alleged negligence against a junior doctor who nasocatheterised a premature baby, resulting in over-oxygenation and blindness. The court held that the doctor's level of experience could not be a defence against an action for negligence. The test was to judge the appropriate standard of competence required for the task inexperience had to be discounted. This case establishes that by extending their role nurses are necessarily taking on greater responsibility, and with this comes greater legal and professional accountability. If a practice nurse engages in tasks that are more suited to the. Plete assessment for patients with panic disorder includes a general medical evaluation as delineated in the Practice Guideline for Psychiatric Evaluation of Adults 15 ; . Such careful assessment, when results are negative, is reassuring for patients who fear serious physical illness. In the case of a patient who is diagnosed with a general medical condition, a decision must be made regarding the relationship between that condition and the panic disorder. If the medical condition or treatment is considered to be involved in the etiology of the panic symptoms, the treatment of panic disorder should be delayed until the general medical condition is treated and or the medication discontinued. However, there may be a general medical condition that is not directly causing the panic disorder. Studies show that panic and other anxiety disorders are more prevalent in medically ill patients than in the population at large. Conditions that have been specifically associated with panic disorder, but not etiologically, include irritable bowel syndrome, migraine, and pulmonary disease 237239 ; . Acute onset of a wide range of medical conditions may also be associated with the development of an anxiety disorder. When a coexisting general medical condition is present, it is important to treat the panic disorder, since panic symptoms may exacerbate the associated general medical condition. Data from some studies suggest that panic disorder and phobic anxiety may increase the risk of mortality in patients with cardiovascular disease 182, 240 ; . In general, treatment of panic disorder in patients with irritable bowel syndrome, pulmonary disease, or migraine is not different from treatment of uncomplicated panic disorder. In fact, studies have documented improvement in respiratory symptoms and amelioration of irritable bowel symptoms upon treatment with SSRIs, even when full-blown panic disorder is not present. If renal or liver damage is present or if there is general debilitation from general medical illness, the medication dose should be adjusted appropriately and repaglinide. 1. The cytoskeleton is crucial in this process with F-actin reorganising to a cortical distribution with stimulation of the cells and abrogation of the pathway when actin is disrupted by Cytocholasin D. Thus we have shown that the podocyte is a novel glucose utilizing, insulin sensitive cell, with actin redistribution in response to insulin and glucose. Because of the known role of F-actin in supporting podocyte foot processes, we suggest that metabolic disturbances of insulin and glucose in diabetes lead to structural foot process alteration, and hence microalbuminuria. Tetracyclineandoxytetracycline have also been reported to cause pseudoporphyria, which manifests with skin fragility, traumatic bullae, scarring and milia formation on sunexposed areas, and normal porphyrin levels. The effective wavelengths for eliciting phototoreactivity fall in the long UV-A and short visible range 320-425 nm ; . Clinical phototoxicity, the rates of photochemical degradation, and the in vitro phototoxicity of the tetracyclines are qualitatively correlated.9 The precise mechanisms of tetracycline phototoxicity are not fully understood. In vitro studies have shown that they are dependent on oxygen and complement. The chromophores in the skin are cell membranes, ribosomal proteins, and DNA.9, 54 Two major types of damage induced in DNA molecules by the photosensitizing activity of tetracycline derivatives are proposed; these are alteration of guanine residues and breakage of the sugarphosphate backbone.54 Quinolones The first quinolone drug, nalidixic acid, was introduced as a uroantiseptic in 1962. Shortly thereafter, a high incidence of photosensitivity reactions was noted among patients treated with nalidixic acid, 55 followed by the description of a UV-Aenhanced photocarcinogenesis in hairless mice.6 Pipemidic acid, oxolinic acid, and rosoxacin, which belong to the same quinolone generation, demonstrated similar photosensitizing properties.18 Currently, the therapeutic use of these quinolone derivatives is limited, although they are still marketed in several countries in Europe. The most frequently reported photosensitive effects of the first-generation quinolones included systemic, often bullous, phototoxic eruptions; porphyria cutanea tarda; and pseudoporphyria. In recent years, the introduction of new fluoroquinolone derivatives raised a great deal of expectation related to greater efficacy at smaller dosages, convenient administration once to twice daily, and diminished toxicity.10, 56 Since the mid. 2541 NRCC 3.2 1974 ; Production of 2, 3-butanediol Can. Over the last twelve months, because of the generosity of supporters like you, the Kennedy's Disease Association KDA ; awarded $75, 000 in research grants. These grants provide critical funding to help researchers focus their attention on finding a treatment or cure for Kennedy's Disease. Of the $67, 242 in donations received by the KDA this past year, 100% was invested directly into the Kennedy's Disease research effort. Unfortunately, the federal government has reduced the funding available for medical research. As a result, research projects for Kennedy's Disease, along with other lesser known, lower public profile illnesses are being seriously under-funded. Research is crucial to finding treatments and a cure for this disease that has affected all of our lives. Because of this, the KDA's goal for 2007 is to award $100, 000 in research grants. Yes, it is an aggressive goal, but not an unreasonable one based upon the generosity you have shown this past year. If we all continue to work together, there is hope that someday soon, we will have a treatment or cure for Kennedy's Disease, for example, oxytetracycline 250.
